SUMMARY:Advancing Residential Aged Care: Quality Improvement workshop - Epping
DESCRIPTION:Primary care clinicians servicing the residential aged care sector are invited to help design a Quality Improvement (QI) program\, which demonstrates the use of SNHN’s residential aged care telehealth platform as part of a hybrid (in-person & virtual) care practice model when caring for residential aged care home patients in the Northern Sydney region. \nThis one-year QI program will include incentive payments upon completion of appropriate Plan-Do-Study-Act (PDSA) cycles. \nThe intended audience of this workshop are GPs\, practice managers\, nurse practitioners\, residential aged care home managers & nurses\, accredited pharmacists and allied health professionals. \nA gift card will be provided to participants as our token of appreciation.
