WEBINAR – Aged Care: New Changes and Essential Support Services

The aged care system is evolving, with significant changes to home support services aimed at improving accessibility and efficiency. Service providers must assess referral pathways, utilising platforms like HealthLink, Care Finder and My Aged Care to streamline aged care assessments.

This workshop will assist GPs and Practice Staff to adapt to the new support-at-home structures and terminology, ensuring informed decision-making in care provision. Maximising the use of Carefinder and Yourside resources enhances service navigation and empowers vulnerable individuals to access tailored support.

By applying strategic approaches to care delivery, Health Professionals can strengthen their ability to advocate for and implement effective aged care solutions.

TARGET AUDIENCE:

GPs, Practice Nurses, Practice Staff and Residential Aged Care Home staff in the Sydney North Health Network Region

LEARNING OUTCOMES:
  • Refer patients to My Aged Care, ensuring an efficient and supportive process
  • Enhance staff awareness of new updates to home support services and assist them in utilising new terminology effectively in practice
  • Utilise Carefinder to improve service navigation and outcomes for clients
  • Utilise knowledge of upcoming changes to CHSP and Short Restorative Care Packages to support older clients
  • Assist at-risk patients in accessing My Aged Care and home support, prioritising equity and inclusivity
COST:
There is no cost for Practices and their staff within the SNHN region to attend this training
