Webinar: Building a Multi Disciplinary Team in General Practice

Multidisciplinary teams (MDTs) are essential in primary care for delivering high-quality, person-centred care that is comprehensive, coordinated, and effective. MDTs can be used for managing chronic conditions, supporting mental health, providing palliative care, and addressing the social determinants of health. This webinar discusses how to build a successful and sustainable MDT model within your practice.

SPEAKERS:

Wendy O’Meara, Practice Manager. She is passionate about General Practice and has a thorough understanding of all facets, with a special interest in MBS and Chronic Disease management. While working full-time, she still takes any opportunities to meet and work with new practice managers, guiding them through the complexities of General Practice.
She has been working as an education consultant since 2017.

TARGET AUDIENCE:

GP’s, Practice Nurses, Practice Managers and Allied Health Professionals

LEARNING OUTCOMES:

1. Describe the benefits of Multi Disciplinary Team care in General Practice
2. Identify the roles and responsibilities of the team members under the new CCM changes
3. Identify and apply funding opportunities including case conferencing, care planning, and the Workforce Incentive Program (WIP)

COST:
There is no cost for GP's, Nurses, Practice Managers and Allied Health Professionals working within the SNHN region to attend this training
