DESCRIPTION:GPs and Practice Nurses\, Allied Health Professionals and Pharmacists caring for older patients with complex needs are invited to an update from Yourside\, the organisation that is providing the Care Finder program in the Northen Sydney region. \nWhat is care finders?\nCare finders help people access aged care services and connect with other relevant supports in the community.\nSome older people need extra\, intensive support to access aged care services and other supports in the community.\nIn these situations\, a care finder may be able to help.\nCare finders is a free service.
