Comprehensive Care for Older Persons: Tackling Chronic Pain, Cognitive Impairment and Palliative Care Decisions

GPs and Practice Nurses are invited to join us at the Geriatrician Outreach to GP education event. The face-to-face event is designed to enhance skills and knowledge in the management of older, complex patients in the community.

The evening will:

  • Enhance knowledge to manage older people and their families, including Chronic Pain, cognitive impairment and Palliative Care decision making
  • Discuss complex case studies with Geriatrician panel. Attendees are encouraged to submit their case studies

Speakers will include Chronic Pain and Palliative Care Specialists from Northern Sydney Local Health District, in addition to the Geriatricians of the Geriatrician Outreach to GP service

The Geriatrician Outreach to GP Service provides specialist input to GP centred care of subacute older and complex patients. The service is part of the Rapid Care for Frail and Older People Program, which includes Keeping Well and Independent Quality Improvement Program (KWIP) and Geriatric Outreach (ARRT, BRACE and GRACE teams).

The program is jointly funded by Northern Sydney Local Health District and Sydney North Health.

  • Staff Specialist from Pain Management and Research Centre, Northern Sydney Local Health District
TARGET AUDIENCE:

General Practitioners and Practice Nurses working in the Sydney North Health Network region.

LEARNING OUTCOMES:
  • Discuss the principles of Geriatrician Outreach to GP service
  • Examine and discuss complex case studies submitted by peers
  • Enhance knowledge in management of patients and their families with mild cognitive impairment, dementia, chronic pain and palliative care
  • Identify resources and services in Northern Sydney to support older patients to ensure a holistic and timely care.
COST:
There is no cost for General Practitioners and Practice Nurses within the SNHN region to attend this training
