Reimagine a healthy community where people can easily access the wrap around care they need, especially those who are at higher risk of poorer health. Join us for a multidisciplinary general practice and allied health dinner and networking event to discuss what is already possible and could be possible with Strengthening Medicare Primary Health Care Reform.  Contribute to the conversation about how access to multidisciplinary team-based care could be improved in our region. The evening is an opportunity to connect with other healthcare providers in your neighbourhood, put faces to names, and learn more about the services they provide.

This networking dinner will be of particular value to health professionals working locally in the Lower North Shore.

TARGET AUDIENCE:

GPs, Registrars, PNs, PMs, Pharmacists and Allied Health Professionals practising in the Lower North Shore.

LEARNING OUTCOMES:
  • Build or enhance multidisciplinary team-based care at their practice (co-located or virtual)
  • Collaborate with other GPs, pharmacists, and allied health professionals
  • Explore current digital technologies to improve two-way sharing of information and communication between health care providers
COST:
There is no cost for GPs, PNs, PMs, Pharmacists and AHPs practising within the SNHN region to attend this networking dinner and training
