SUMMARY:Face to Face: Improving Outcomes for Patients with Dementia
DESCRIPTION:We welcome all General Practice teams to attend this dinner meeting. \nDementia Australia will provide an overview of education\, services and support available to people living with dementia\, and there will be presentations on a range of quality improvement initiatives in general practice\, including an Ageing Well Screening Tool. It will also be an opportunity to share and discuss dementia care and quality improvement strategies\, successes and challenges\, with other practices with an interest in dementia.
