Making the link between kidney disease, diabetes and other chronic conditions

This workshop will look at new approaches in kidney disease management, current guidelines and the relationship between poor kidney function and other chronic conditions including diabetes, CVD and the risk of dementia. It will also address nutrition management in patients with impaired kidney function.  We will explain how to look at your renal data and implement relevant quality improvement activities in general practice.

Local General Practitioners, Allied health Professionals, Credentialled Diabetes Educators and Practice Nurses

COST:
There is no cost for GPs, PNs, Credentialled Diabetes Educators, and Allied Health Professionals working within the SNHN region to attend this workshop.
