The last months of life: end of life care support

General Practitioners are increasingly providing palliative care and end of life care support as our population ages. GPs can feel a lack of confidence in managing patients at end of life due to lack of training.  This education event aims to provide an update on:

  • supports available in the community including community palliative care, Northern Sydney Home Nursing Service, Hospital in the Home, Navigator service, Caring Communities, care packages
  • update on end of life symptom management
  • update on Voluntary Assisted Dying (VAD) and the interface with community services, paperwork at end of life (including VOD/MCCD/cremation paperwork update/coroners requirements)
  • Review of opioids used at the end of life, and rules and regulations around their use
  • challenges of prognostication in malignant and non-malignant disease

This event will be held at the NSLHD Adolescent and Young Adult Hospice (AYAH) and will also include a panel discussion with GPs, Northern Sydney Home Nursing representative and palliative care representative on how we can best work together to provide optimal care for our patients.

TARGET AUDIENCE:

LEARNING OUTCOMES:
  • Describe an evidence-based approach to management of symptoms at the end of life
  • Identify opioids used at the end of life, issues with availability, and the rules and regulations surrounding prescribing opioids
  • Explain the multiple teams and services that can be provided to a patient at home, including options such as syringe drivers, ports and PleurX
  • Describe the paperwork available to support a patient at home for end of life, and after death
  • Discuss the workings of the Voluntary Assisted Dying team, and how to refer a patient requesting Voluntary Assisted Dying
COST:
There is no cost for General Practitioners within the SNHN region to attend this training
