SUMMARY:Face to Face: Primary Care Reforms and Quality Improvement General Practice
DESCRIPTION:GPs and Practice Care Teams are invited to attend this education session providing the latest information on the Strengthening Medicare and the budget and how you can embrace the changes to improve your current practice and patient outcomes. We will explore how to use data and team to inform quality improvement and improve patient management and care. \nThe education session will be facilitated by Dr Walid Jammal\, principal GP with a keen interest in health economics and models of care in general practice\, especially the Patient Centred Medical Home framework; and Dr Kirsten Meisinger\, MD who is an international expert on Patient Centred Medical Homes and healthcare system transformation. \nParticipants will learn and explore the principles of team-based care and their application in General Practice as well as aligning this to the use of quality improvement tools and techniques. \nLunch will be provided between 12-12.45pm
