Integrating On-site Pharmacists into Residential Aged Care Homes

Integrating on-site pharmacists into residential aged care homes aims to enhance medication management and safety for residents by embedding pharmacists within care teams.

This Australian Government initiative focuses on improving the use of medications, especially high-risk ones, and ensuring continuity in medication management through regular reviews and prompt issue resolution. Eligibility criteria include specific requirements for both aged care homes and pharmacists, such as accreditation and training.

On-site pharmacists will perform various activities, including medication reviews, providing advice to residents and staff, and collaborating with other healthcare professionals to optimise health outcomes.

  • The eligibility criteria for the aged care home and the pharmacist
  • The scope of activities an onsite pharmacist can perform

  • Evaluate the effectiveness of the Aged Care Onsite Pharmacist Initiative.
COST:
There is no cost for Practices and their staff within the SNHN region to attend this training
