SUMMARY:Face to Face: The Last Days Program
DESCRIPTION:The Last Days Program is a 3 hour workshop delivered by an experienced Palliative Care Nurse\, tailored for family and friends. It equips participants with practical tools and information to help navigate a dying person’s last days. Be quick to register as spaces are limited – There is NO cost to attend this community education event. \nLast Days was developed as part of a collaborative initiative of Sydney North Health Network and HammondCare. The workshops are facilitated by HammondCare and will be delivered by Sydney North Health Network under the Greater Choice for at Home Palliative Care initiative\, funded by the Department of Health and Aged Care. \nPrivacy statement: The registration information we collect is provided to HammondCare for the purpose of delivering the workshops\, as well as evaluation and reporting of the Last Days program. The security of your personal information is very important to us at Sydney North Health Network. \nNOTE: There are multiple dates and times available\, see options under register.  \nLight refreshments will be provided at the workshop – for catering purposes\, please ensure that each participant is individually registered to attend.
