WEBINAR: Healthy End of Life Planning (HELP) App for the Palliative Care Workforce

Palliative Care services are essential for delivering the best care for people at end of life but we can’t do it all. We need to encourage our clients and their families to have a system in place to access additional support from their friends and community.

The Healthy End of Life Planning (HELP) App is a free resource designed to coordinate this support for people in palliative and aged care settings.

SPEAKERS:

Holly Smith – Public Health Palliative Care Unit, La Trobe University
Holly Smith has worked in the palliative care sector for 6 years with a focus on Public Health Palliative Care and championing early Compassionate Communities work. She now works at the Public Health Palliative Care Unit at La Trobe University where they have been researching how to best support people to die well in their place of choice. This has culminated in the development of a palliative care social prescribing model and the HELP App which is improving end of life experiences for people around Australia.

Sally Brown – Public Health Palliative Care Unit, La Trobe University
Sally has been working in palliative and end-of-life care since 2007 and is currently a Partnership Coordinator with the Healthy End of Life Program at La Trobe University’s Public Health Palliative Care Program. Sally trained as a Radiation Therapist and worked in clinical and research posts before moving into the academic space as a Clinical Tutor, Senior Lecturer, Course Coordinator and Stream Leader both in the UK and Australia.

TARGET AUDIENCE:

GPs, Nurses, Allied Health Professionals, Aged Care Health Professionals, Staff and Palliative Care workforce across the Sydney North Health Network region

LEARNING OUTCOMES:

Participants will:

  • Gain a strong understanding of network centred care and how this improves end of life experiences
  • Gain insight into palliative care social prescribing and the HELP App
  • Confidence in delivering the social prescribing model to patients and families as part of routine practice
COST:
There is no cost for anyone in the palliative care workforce, practices and their staff in the Northern Sydney Region.
