DESCRIPTION:The Lumos program has been adopted in a partnership between Sydney North Health Network (SNHN) and the NSW Ministry of Health. It aims to assist general practices in gaining a stronger understanding of their patients’ journey across the health system. Lumos Practice reports can help identify opportunities for improving patient outcomes and experiences. \nThe Lumos program has been funded by the Commonwealth Government under the Health Innovation Fund. There is no cost for General Practices to participate.
