Advancing Residential Aged Care: Quality Improvement workshop – Epping

Primary care clinicians servicing the residential aged care sector are invited to help design a Quality Improvement (QI) program, which demonstrates the use of SNHN’s residential aged care telehealth platform as part of a hybrid (in-person & virtual) care practice model when caring for residential aged care home patients in the Northern Sydney region.

This one-year QI program will include incentive payments upon completion of appropriate Plan-Do-Study-Act (PDSA) cycles.

The intended audience of this workshop are GPs, practice managers, nurse practitioners, residential aged care home managers & nurses, accredited pharmacists and allied health professionals.

A gift card will be provided to participants as our token of appreciation.
