Frailty and Falls is Everyone’s Business

Frailty is a major concern for older adults, significantly increasing their risk of falls. Interestingly, the level of risk can vary depending on the frailty assessment tool used, with the FRAIL scale indicating the highest risk.

In this session, we’ll explore various interventions that health professionals can implement to help mitigate these risks and improve the quality of life for our elderly patients.

TARGET AUDIENCE:

GPs, Primary Care Nurses, Pharmacists and Allied Health Professionals and their staff from the Sydney North Health Network region

LEARNING OUTCOMES:
  • Describe and implement best practice fall prevention strategies based on latest research
  • Design individualised and group-based exercise programs that improve strength and balance, and help people stay active doing the things they love
  • Define the role and importance of nutrition as a modifiable risk factor of frailty
  • Utilise services and resources to support older patients to engage in health and lifestyle modifications that reduce their risk of frailty and falls
  • Utilise a multidisciplinary team approach in supporting patients at risk of falls
COST:
There is no cost for participants from the target audience and their staff within the SNHN region to attend this training
