WEBINAR: Quality Improvement to enhance patient care in General Practice – The Lumos Program

The Lumos program has been adopted in a partnership between Sydney North Health Network (SNHN) and the NSW Ministry of Health. It aims to assist general practices in gaining a stronger understanding of their patients’ journey across the health system. Lumos Practice reports can help identify opportunities for improving patient outcomes and experiences.

The Lumos program has been funded by the Commonwealth Government under the Health Innovation Fund. There is no cost for General Practices to participate.

SPEAKERS:

Dr Walid Jammal | GP – MB BS (Syd), Dip Child Health, FRACGP, MHL

Dr Jammal is the principal GP in a well-established teaching general practice in western Sydney. His special interests include quality and safety in health care and health reform. He was a member of the Strengthening Medicare Taskforce.

TARGET AUDIENCE:
Local GPs, Practice Managers, and Nurses are invited to participate in the webinar on Utilisation of LUMOS reports in General Practices.
LEARNING OUTCOMES:
  • Explain the importance of data in Primary Care for better health outcomes including preventive care, early intervention, and personalised treatment plans
  • Critically assess current and emerging population health needs using Lumos reports
  • Plan and design Quality Improvement initiatives using LUMOS reports to enhance patient outcomes in General Practices
COST:
There is no cost for Local GPs, Practice Managers, and Nurses within the SNHN region to attend this training
